Emergency Contacts

The school must have correct home address with proof of residency and at least two telephone numbers.  If students become sick or hurt during the school day, parents may need to be called. We need accurate contact information at all times. Please keep your child’s teacher and office informed of any changes that occur in addresses, phone numbers, and emergency contact persons.
